If you are unable to upload, please mail your request to our Request Processing Center located at 1142 S. Diamond Bar Blvd #310, Diamond Bar, CA 91765.

You can always check the status of your record request from our Online Status Portal, all you need is your unique Reference Number and the patient's Date of Birth. CHECK THE STATUS OF YOUR REQUEST NOW!
As soon as your request is completed, we will notify you by email. You can download your file as many times as you need for ONLY FIVE 5 DAYS, from our email notification date.
For security reasons, your file will be destroyed from our servers, after this period of time. If for whatever reason, a file needs to be re-uploaded, there will be a retrieval process fee of $25.
